Innovation Fund

Herold Road Family Physicians was one of twenty-eight medical clinics across Saskatchewan to have received support from the Innovation Fund. The Innovation Fund was established to help clinics better meet the needs of their patients and communities by establishing physician-led, team-based care. 

This project, led by Drs. Chaloupka, Davis, Sathi, and Symon, aims to improve access to mental health care services, dietary counselling, enhance care coordination and expand team-based care for adults diagnosed with chronic diseases. Through adding social workers, a dietitian and RN to our team, the goal is to have people available with the correct skills to meet the needs of our patients right in the Patient’s Medical Home, Herold Road Family Physicians.
