Rae Rivard

Rae has worked in chronic disease management for over 25 years, with a primary focus on diabetes care.

She is a Registered Dietitian and Certified Diabetes Educator who practices patient-centered care, recognizing that health is influenced by more than nutrition alone. Her approach is grounded in the belief that all foods can fit within a balanced lifestyle, and she emphasizes realistic, sustainable goals to support long-term health outcomes. She is passionate about supporting individuals in developing the knowledge and confidence needed to effectively manage chronic conditions.

Outside of her professional practice, she values time spent with her husband and four children, particularly at their family cabin, and enjoys staying active outdoors.

Jenn Bauer

Jenn is a Clinical Social Worker based in Saskatoon. She holds a Bachelor of Arts in Psychology from the University of Saskatchewan (2004), a Bachelor of Social Work from the University of Regina (2006), and a Master of Social Work from Dalhousie University (2015).

Jenn has worked in a variety of medical and mental health settings, providing direct client care and supervising multidisciplinary teams. She began teaching with the Faculty of Social Work at the University of Regina in the summer of 2024. In 2024, Jenn was fortunate to be part of the Collaborative Mental Health Care Project at Herold Road Family Physicians and is thrilled to be returning to the team!

Jenn has additional training in several evidence-based therapeutic approaches, including Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), Motivational Interviewing, Narrative Therapy, and strengths-based and solution-focused interventions.  She supports clients with a wide range of mental health concerns and has a particular interest in childhood trauma. Jenn has completed Phase I Certification in the Neurosequential Model of Therapeutics through the Child Trauma Academy in Houston, Texas, and brings a trauma-informed, compassionate approach to her work.

Outside of work, Jenn enjoys reading and stays busy keeping up with her kids’ activities.
